SEAFARER CERTIFICATION INSTRUCTIONS

Instructions for completing the following forms may be found in the MI-118, Republic of the Marshall Islands Requirements for Merchant Marine Personnel Certification, which is a part of the MI-300, Republic of the Marshall Islands Combined Publications Folder. Contact any IRI Office for a copy.

Although the MI-118 has application forms attached, they are not set up to be used on-line. You may print out what you need and fill it in manually. If you want to print out a certain page, either place your cursor on the page you want and request 'current page' when in the print window or type in the numbers of the pages you want in the 'Pages' box in the print window. If you want to be able to fill out the forms on-line, you will need to use the forms in this section.

MI-105 Application for Endorsement of Officer Certificate of Competence

MI-105E Application for Original Officer Certificate of Competence by Examination

MI-105M Physical Examination Report/Certificate

This Physical Examination Report/Certificate must be completed by a licensed physician and submitted with:

  • Application for Officer Certificate of Competence; OR

  • Application for Seafarer's Identification and Record Book and Special Qualifications.

MI-116 Affidavit of lost or destroyed license.

MI-273 Application for Seafarer's Identification and Record Book and Special Qualifications

MI-273I Instructions for Application for Seafarer's Identification and Record Book and Special Qualifications

MI-279P Application for Certification of Special Qualifications (Pax Vsl)

This application form is used to apply for Marshall Islands Special Qualification as Survival Craft/Rescue Boat Crewman for the "hotel staff" on passenger vessels.
